Family Assistance Program

The CHARGE Syndrome Foundation Family Assistance Program (FAP) was created in the Spring of 2020 in response to increased needs of families related to the COVID-19 pandemic.  Since its inception, the Foundation has supported over 100 families across the United States.

MEDICAL TRAVEL ASSISTANCE PROGRAM

This needs based program may be used to cover transportation, meals, lodging, and parking for medical care that requires out of state/long distance travel. The program is intended for families who would be unable to travel for medical care without this assistance. We hope this program will alleviate some of the financial burden.

This is a need based program for U.S. families only.

Crocker Barker CHARGE Sibling Scholarship

Thanks to a generous donation from the Barker family, the Crocker Barker CHARGE Sibling Scholarship was created to help siblings of individuals with CHARGE attend post-high school education such as trade school, community college, or university.

Application deadline: June 15, 2021; Awards made by July 15, 2020
Applicants must reside in the US or Canada.

 

 

  • Awards start at $2,000.
  • Financial need must be demonstrated.
  • Awards will be used to pay for tuition, fees, and/or books.
  • Applicant must be enrolled at least part-time.
  • One time only award payable to post secondary educational institution of recipient’s choice.

WELCOME PACKAGE

The CHARGE Syndrome Foundation offers new families direct support and resources as they navigate the complex needs of an individual with CHARGE.  As part of our welcome to the CHARGE community, our Family Assistance Program provides a special Foundation welcome package to families of individuals with CHARGE who have recently been diagnosed.  Note: Family must reside in the US to receive this package.
